What is Goldfields Mental Health Portal?
Brought to you in collaboration with Centrecare, Goldfields Women’s Health Care Centre and funded by the Western Australia Primary Health Alliance (WAPHA), the Goldfields Mental Health Portal will deliver place-based low intensity support and psychological interventions to people of the Goldfields-Esperance region. Working within a Stepped-Care Model; your needs will be matched to the level of mental health support and care you require. With highly qualified and trained professionals, you can be confident that your journey to recovery will be tailored to your needs.

What is a Stepped Care Model?
Basically, the Stepped-Care Model takes into consideration the exact type of intervention that you need and you can access any level at any time should you require more intensive or less intensive support. The Stepped Care Model splits into three categories:

  1. Integrated Care Management (ICM)
    Through a thorough assessment of your needs and working together with you to create a recovery plan with input from your GP, our ICM Care Coordinators and Case Workers will provide one to one therapeutic approaches to brief interventions and education, help to coordinate your care with other services to help minimise attending too many different appointments, link you in with a counsellor if and when this is needed and help you to get involved with low intensity group work to grow your network and build your confidence.

  2. Community Services Supports (CSS)
    Through a thorough assessment of your needs, and working with you to create your recovery plan, our CSS Case Workers will work primarily in an outreach and group support setting, providing you with the help you need through home visits and engaging you with community support networks where you can build your friendship circle as well as your own skills, awareness and confidence. We will support you throughout the Goldfields, if you live in a rural or remote area, our Case Workers will come to you. If needed you will receive brief and focused mental health specific activities and be linked in with a counsellor if applicable.

  3. Practitioner Online Referral Treatment Service (PORTS)
    PORTS is an innovative online/telephone based treatment service that offers brief telephone Strengths Based Therapy (SBT) and Cognitive Behavioural Therapy (CBT) courses and structured online treatment courses. This means that you can access support wherever you live or are based and at whatever time suits you. PORTS will be offered to clients that have mild anxiety or depression symptoms and our Case Workers can support you in accessing the Portal from your home or from one of our offices across the region. If you need further one to one or group support, the Goldfields Mental Health Portal will link you to the appropriate stream.

If at any point you feel your symptoms are getting better or worse, the Goldfields Mental Health Portal will adapt your recovery plan to ensure that you are getting the appropriate level of service. With ongoing assessments and reviews, and with consistent conversations with your GP, you can be confident that the Goldfields Mental Health portal will support you in a holistic and responsive way to ensure that you get the best out of your time with the service.
