What is Goldfields Mental Health Portal?
Brought to you in collaboration with Centrecare, Goldfields Women’s Health Care Centre and funded by the Western Australia Primary Health Alliance (WAPHA), the Goldfields Mental Health Portal will deliver place-based low intensity support and psychological interventions to people of the Goldfields-Esperance region. Working within a Stepped-Care Model; your patients’ needs will be matched to the level of mental health support and care that they require. The Goldfields Mental Health Portal aims to coordinate your patients’ care to reduce the burden on the individual having to access multiple services and receiving multiple assessments and to minimise the over or under-servicing of individuals in the region. With highly qualified and trained professionals, you can be confident that your patients’ journey to recovery will be tailored to their needs.

What is a Stepped Care Model?
Basically, the Stepped-Care Model takes into consideration the exact type of intervention that your patient requires which they can access any level at any time should they present with more or less intensive mental health concerns. The Model also provides opportunities to share information about your patient to influence their treatment pathway and to ensure that the patient has a transparent recovery plan that incorporates their medical treatment as well as their psychological needs. Our Clinical Lead and Care Coordinators will report your patients’ progress to you on a regular basis and updates will be included in case management meetings where a client is receiving shared care from multiple agencies. The Stepped Care Model splits into three categories:
- Integrated Care Management (ICM)
ICM will work with individuals that have severe and complex mental health issues, giving the individual dedicated time and support to build their capacity. A Case Coordinator will work with High and Complex needs individuals intensively through care coordination between GPs and services needed to aid recovery and individual support, inclusive of brief interventions, psychoeducation and psychotherapies; with referrals to counselling and other services as needed. ICM will provide the above high intensity care coordination and individual support whilst also providing the individual with opportunities to engage with community supports including group support and workshops. The intended timeframe of an individual engaging with ICM is three months
- Community Services Supports (CSS)
Should a patient be deemed to have medium mental health needs; the client will be entered into Community Support Services where individual, group support and care coordination will be offered, however this will be defined on a lower scale of intervention with a higher emphasis on group support; encouraging self-efficacy, raising self-esteem and encouraging community engagement and belonging.
The CSS case worker will create recovery plans with the client and identify any precipitating factors that may have an impact on the client’s mental health and wellbeing, as well as their recovery journey; referring to appropriate streams if necessary should the client need higher or lower intensity interventions. It is anticipated that clients will be engaged with CSS for a period of three to six months, at which point they will be referred to either an internal or external mental health service or exited from the service.
- PORTS (Practitioner Online Referral Treatment Service)
PORTS is an innovative online/telephone based treatment service that offers brief telephone Strengths Based Therapy (SBT) and Cognitive Behavioural Therapy (CBT) courses and structured online treatment courses. Your patient can access support wherever they live or are based and at whatever time suits them. PORTS will be offered to clients that have mild anxiety or depression symptoms and our Case Workers can support your patient in accessing the Portal from their home or from one of our offices across the region.
To be entered on to the PORTS stream, clients will either have the ability to self-direct their treatment without the need to have a Case Worker intensely directing them through the service, will be based in a remote area where access to face-to-face support is difficult, or will have low level mental health concerns that can be addressed through brief online or telephone support.
PORTS clients will still be offered the support of a case worker to help navigate referral pathways and to provide information and place-based support if necessary. Clients are also encouraged to join groups and workshops to enhance their recovery and promote community engagement and belonging. Clients will be monitored throughout their PORTS journey and the PORTS team will report the clients’ progress to GPs and Medical Practitioners.
Clients will be offered the opportunity to engage with CSS or alternate mental health services if applicable upon completion of their PORTS intervention.
